INTRODUCTION: Intracholecystic papillary neoplasm (ICPN) is a recently identified disease characterized by papillary pre-invasive neoplasm of the gallbladder. Despite its characterization, the natural history of ICPN remains elusive. Furthermore, a few cases of metachronous ICPN in the remnant biliary system have been documented. Here, we report a surgical case involving metachronous ICPN in the remnant cystic duct 19 years post-cholecystectomy for primary ICPN.

INTRODUCTION: Gallbladder metastases from malignancies, including gastric cancer, are extremely rare. Diagnosis is challenging due to the lack of specific symptoms and the absence of distinctive radiological features that can differentiate metastatic lesions from primary gallbladder tumors.

INTRODUCTION: Esophageal schwannomas are rare benign tumors arising from Schwann cells and are among the least common mesenchymal neoplasms of the gastrointestinal tract. While fluorodeoxyglucose positron emission tomography (FDG-PET) is widely used in the preoperative assessment of submucosal tumors, schwannomas are known to exhibit unexpectedly high FDG uptake, often mimicking malignant lesions. Dual-time-point FDG-PET, which evaluates both early and delayed FDG accumulation, has been employed to improve diagnostic specificity in various malignancies, including esophageal cancer. However, to date, no cases of esophageal schwannomas showing a malignant FDG uptake pattern on dual-time-point FDG-PET and resected thoracoscopically have been reported. We present a rare case of esophageal schwannoma demonstrating increased delayed FDG uptake, initially suggestive of malignancy that was successfully treated with thoracoscopic surgery following preoperative diagnosis via EUS-FNA.

INTRODUCTION: We have performed approximately 200 colorectal cancer surgeries using Senhance Surgical System in our department and we have standardized the surgical procedure with Senhance. For the first time, we report the case of lateral lymph node dissection for recurrence of rectal cancer with Senhance Surgical System.

Coexisting Remnants of the Omphalomesenteric Duct and Urachus in an Infant
INTRODUCTION: Congenital anomalies of the umbilicus, including remnants of the omphalomesenteric duct and urachus, result from the incomplete regression of fetal structures around the 10th week of gestation. The coexistence of these anomalies in a single patient is exceptionally uncommon. This report presents the case of a neonate with an umbilical nodule and periumbilical cyst, subsequently identified as coexisting remnants of the omphalomesenteric duct and urachus.

INTRODUCTION: Although complete resection during radical surgery is a crucial prognostic factor for locally advanced colorectal cancer, achieving it is often difficult when the tumor invades the iliac artery system. Herein, we report a successful case requiring resection of the common iliac vessels and vascular reconstruction using a femoral–femoral arterial bypass (F–F bypass), with a comparison to 4 previous cases involving F–F bypass.

INTRODUCTION: Laparoscopic sleeve gastrectomy is a standard and safe surgical procedure for patients with morbid obesity. A potential complication is piriform fossa injury, which can occur during calibration tube insertion. We encountered a case of descending necrotizing mediastinitis, a serious and progressive infection originating from a piriform fossa injury that was potentially exacerbated by diabetes.

INTRODUCTION: An epiphrenic esophageal diverticulum (EED) typically occurs in association with esophageal motility disorders such as esophageal achalasia. Although peroral esophageal myotomy (POEM) is the current standard treatment for esophageal achalasia, laparoscopic diverticulectomy with esophageal myotomy and fundoplication are usually performed for EED. Here, we report four cases of thoracoscopic esophageal diverticulectomy performed for symptomatic EED after POEM for esophageal achalasia.

INTRODUCTION: Achalasia is a primary esophageal motility disorder of unknown origin. The clinical manifestations are caused by the loss of peristalsis of the esophagus and functional obstruction at the esophagogastric junction. There are several treatment strategies for esophageal achalasia, such as medications, endoscopic treatment, and surgery. The successful treatment of a case of jejunal interposition surgery with overlap esophago-jejunal anastomosis for an esophageal stricture due to repeated endoscopic dilation for esophageal achalasia is reported.
